FAQ: WHAT MUST I PROVIDE TO THE LIVESCAN SERVICE PROVIDER I CHOOSE?

Provide the following to the LiveScan service provider:

  1. All applicants will be required to bring two (2) forms of identification to the electronic fingerprinting site on the day of fingerprinting. One of the two types of identification must bear your picture and signature such as a driver’s license, state identification card or passport.
  2. If you are an applicant seeking a license for any profession regulated by the Department of Health which requires a criminal background search as a condition of licensure, you must provide accurate demographic information at the time your fingerprints are taken, including your Social Security number. If you are unable to legally obtain a social security number, you must provide an individual taxpayer identification number (ITIN). Please be sure to provide your profession’s ORI number and other demographic information.
  3. You must provide the correct ORI number. If you apply online, there is a form that you can print out that will have your ORI number prepopulated. In the paper application, you can locate your ORI number in the instructions. A complete list of ORI numbers can also be found on this site under “Screened Professions” on the Screening Requirements tab.
  4. In addition to compliance with the Florida Department of Law Enforcement transmission requirements, the Livescan Service Providers will need the following information in order for the Department to receive the results appropriately. Failure to submit this information may result in the Department’s inability to obtain screening results:
    1. Full Name
    2. Address (the individual’s address is needed for the Agency to make contact if necessary)
    3. Social Security Number (or Tax ID Number. Section 435.04(1)(e), Florida Statutes)
    4. Date of Birth
    5. Race
    6. Sex
    7. Weight
    8. Height
    9. Weight
    10. Hair Color
    11. Eye Color
    12. Place of Birth
    13. Employer Name
    14. Employer Address
    15. Screening ORI Number
  5. Note: Although the ITIN will be sufficient to complete the background screening requirement, you will still need a social security number to be licensed by our agency. A social security number is a statutory requirement for licensure. You will not be granted a license until you are able to provide the Department with a valid social security number. If this information needs updating, you will need to mail a copy of your social security card to the appropriate board office for update.
